Free Employee Evaluation Forms Printable

It’s time for some employee evaluations, and I’ve found some free printable forms to make the process a little easier. Check out these options:

Evaluation Form #1

Evaluation Form #1This first form is pretty basic, but it gets the job done. It has fields for the employee’s name, department, job title, and date of evaluation. Then, there are various sections where the evaluator can rate the employee’s performance on things like job knowledge, communication skills, teamwork, and attendance. There’s also space for comments and recommendations.

It has sections for the employee to self-assess their job performance, as well as sections for the supervisor to evaluate the employee’s job knowledge, work quality, communication skills, and other factors. There’s also space for additional comments and overall ratings.

It has sections for rating the employee’s performance on specific job duties, as well as an overall rating section.
